The clinical experience with sildenafil and other drugs with vasodilatory effects is limited. The effect of sildenafil, a vasodilator, on the sildenafil-induced increase in plasma levels of cGMP has not been studied. This is the first study investigating the effect of sildenafil, a phosphodiesterase-5 inhibitor, on sildenafil-induced prostaglandin formation in vitro. Sildenafil (50, 100, 150, and 200 micrograms/ml) and Ciprofloxacin (3.5, 5, 10, and 15 micrograms/ml) were tested in vitro. Sildenafil, with a dose of 50 micrograms/ml, significantly inhibited the sildenafil-induced increase in cGMP (mean +3.0 +/- 0.5 vs. 0.3 +/- 0.1; p < 0.01). The prostaglandin formation in the sildenafil-treated group was significantly higher than that in the control group (3.9 +/- 0.2 vs. 2.2 +/- 0.4; p < 0.05), whereas no statistically significant difference was found in the prostaglandin formation in the Ciprofloxacin group (3.4 +/- 0.3 vs. 2.1 +/- 0.6; p < 0.05). Our study suggests that sildenafil, a vasodilator with a dose of 50 micrograms/ml, may cause an increased prostaglandin formation in the corpus cavernosum of rats. This effect is not mediated by prostanoids but by the formation of prostaglandins in the corpus cavernosum.

The safety and efficacy of sildenafil (Viagra), a phosphodiesterase type 5 inhibitor, has been demonstrated in clinical trials, and the safety and efficacy of sildenafil has been demonstrated in clinical practice []. Viagra was originally developed to treat erectile dysfunction (ED) and is widely used to treat pulmonary arterial hypertension (PAH), an abnormal heart rhythm condition characterized by abnormal heart rate and blood pressure. However, its use has not been demonstrated to be beneficial in patients with ED, particularly when they are treated with nitrates and other potent vasodilators [,]. Since its approval in 1998, sildenafil has become a go-to for the treatment of ED and PAH in general []. The use of sildenafil has been reported to cause a decrease in the number of sexual events during clinical trials of sildenafil (Viagra), a phosphodiesterase type 5 inhibitor. The clinical trial of sildenafil showed a significant decrease in the rate of sexual intercourse in the treated group and an increase in the frequency of erections and orgasmic events, as assessed by the International Index of Erectile Function []. Sildenafil is a selective inhibitor of the PDE5 and PDE5/PDE-5 receptors, which inhibit the activity of both PDE5 and PDE-5, and therefore have a potential to decrease the incidence of cardiovascular events such as heart attack, heart failure, and stroke [].

A South Korean transgender woman has been jailed for five years after pleading guilty to a charge of criminal offences related to the use of counterfeit sexual performance enhancement pills and distributing unapproved drugs.
The defendant, Choi Soon-sil, pleaded guilty to a charge of criminal offences related to the sale of counterfeit goods.
The sexual enhancement products, including Viagra and Cialis, were seized by the Department of Health, and by the National Bureau of Investigation, South Korea, on March 25.
Prosecutors said the products were distributed by a supplier of the counterfeit products in South Korea to a male and a woman at a house in the province of Kwanjum, near the town of Haei-dong.
Prosecutors said that at least eight men and four women purchased the fake erectile dysfunction pills for the purpose of engaging in sexual acts, including masturbation and oral sex.
They also found that the pills had been misbranded and the tablets were not properly labelled.
Prosecutors said that the defendants were involved in a multi-million dollar criminal enterprise that included the sale of counterfeit goods, distribution of unapproved drugs and the possession of counterfeit medicines.
The goods were distributed in South Korea by the Ministry of Health and Prevention of crime and were not approved by the South Korean regulatory agency for sale.
The products were seized in Kwanjum province, the Department of Health said in a statement.
